Zollinger-Ellison syndrome: 1. Paradoxical postprandial inhibition of gastric secretion1

چکیده
The gastric acid, pepsin, and secretory volume output in response to a mixed meal were measured in six patients with Zollinger-Ellison syndrome caused by a gastrin-producing tumour proved subsequently at surgery. The patients were all normocalcaemic, and none had previous abdominal surgery. In four of the six patients, ingestion of the meal markedly inhibited the gastric secretory output, which decreased to below fasting levels, returning later to basal values. In two other patients, whose fasting acid output was considerably lower, the secretory output increased after the meal, but some inhibition of gastric secretion was also apparent for variable intervals of time. The serum gastrin concentration in all patients remained essentially unchanged or increased after the meal. Two patients were restudied after successful removal of the duodenal gastrin-producing tumour, and in each the normal gastric secretory and gastrin-releasing

Pathophysiological responses to meals in the Zollinger-Ellison syndrome: 2. Gastric emptying and its effect on duodenal function.
In this study, we investigated the relationship between gastric emptying and duodenal events in patients with the Zollinger-Ellison syndrome due to a gastrinoma. Like the inhibitory effect of a meal on gastric secretion (described in a companion paper), postprandial inhibition of gastric emptying reduces fractional gastric emptying rates to normal during the first two hours after a meal. Gastri...
متن کاملZollinger-Ellison Syndrome — Presentation, Diagnosis, Therapy
Zollinger-Ellison syndrome results from a pancreatic tumor in the non-beta islet cells, which stimulates the maximal activity of the gastric acid-secreting cells, resulting in gastrointestinal ulceration of the mucosa. Multiple endocrine neoplasia type 1 (MEN 1) is a familial autosomal dominant syndrome that includes Zollinger-Ellison syndrome as part of it; however, Zollinger-Ellison syndrome ...

Ellison described the classic triad of peptic ulcer disease, gastric acid hypersecretion and islet cell tumors of the pancreas that constitute the syndrome that bears their names (1). Most patients with Zollinger-Ellison syndrome (ZES) require long-term medical management to suppress gastric acid hypersecretion, control sympto...

A B S T R A C T Gastric emptying and secretion, as well as intragastric volume and composition, were determined simultaneously in three patients with Zollinger-Ellison syndrome and in seven normal subjects. Gastric hypersecretion was observed in patients with ZollingerEllison syndrome and in normal subjects receiving pentagastrin. In contrast, the fraction of gastric contents emptied per minute...
متن کاملIsolation of two minigastrins from Zollinger-Ellison tumour tissue.
A pair of gastrin tridecapeptides (;minigastrins') have been isolated from Zollinger-Ellison tumour tissue; they correspond to the fragment 5-17 of the heptadecapeptides isolated from the same source. In one (type II) the tyrosine residue present is sulphated, in the other (type I) it is not. The proportion of types I and II is approximately 2:1 similar to that for the ;big' gastrins and the he...
